How to complete the review
When the review begins

What you’ll need to complete the review
To complete the review, you'll need:
- your National Insurance number (if you do not have this, it could take longer to complete your review)
- the bank account details where the child's benefits are paid into
- details of any changes you're telling Social Security Scotland about
- the date the changes happened, or will happen
You can find your National Insurance number on your:
- National Insurance card
- benefit letter
- payslip
- P60

You'll also need the child’s Child Reference Number, if they have one. This number is sometimes known as a CRN. It could be on a benefit letter or decision letter if you have applied for Child Disability Payment or Disability Living Allowance for children before.
If you're acting on someone’s behalf
If you're the appointee for someone who cannot manage their own affairs, call Social Security Scotland free on 0800 182 2222. Find out what to do if you need help contacting Social Security Scotland, or are calling from abroad.
Before you complete the form, call Social Security Scotland free on 0800 182 2222 if:
- you do not have a permanent address
- you're moving to an address outside the UK
- you do not have a bank account and want to be paid in the way you have agreed with the Department for Work and Pensions
- you want to report a change in your payment details
- you're reporting a change for someone who cannot manage their own affairs and you're their appointee
- you want to report the death of a child
