How to get a paper PVG application form

The quickest way you can apply for basic disclosures and apply for PVG is online.

You can register your organisation with Disclosure Scotland if you're applying for higher level disclosures for your workers.

Paper application forms

If you cannot complete a PVG application online, you can ask Disclosure Scotland for a paper application form. This takes longer, so try to apply online first.

Email response@disclosurescotland.gov.scot to get paper forms. Include your:

name
address details
the number and type of application form you need
