How to apply for care equipment or home adaptations

If you or someone you care for needs specific support, contact your local council’s social work team. They will arrange a care needs assessment. 

A social care professional will visit you to:

  • talk about your situation
  • find out what help you need
  • explain what support they can offer

What happens at a care needs assessment

A social care professional will arrange to meet you to do the assessment in your home.

You can also have a family member or friend with you at the assessment to support you.

The assessor will ask you questions about:

  • what you can do
  • how you get around your home
  • what you have difficulty with

They will also discuss any equipment you might need with you.

The information from the assessment goes into a care plan. Your care plan sets what support the council or NHS can offer, and how they plan to meet your needs. You have the final say in what happens with your care.

Financial help with home adaptations

You may be able to get financial help for changes to your home if:

  • you, or someone in your home is disabled, or has problems getting around
  • it needs changed so that it meets your needs, or the needs of someone you live with
  • the changes are essential
  • you need help paying for the work

Support will vary depending on where you live. Each local council has their own policies on what help they can provide. They will tell you more during your care needs assessment.
