Referral form for regulatory bodies and integration joint boards
Form used by regulatory bodies and integration joint boards to make a referral to Disclosure Scotland. It can also be used by councils in certain cases.
Information for councils
If the referral is about a person who is not employed by you because you found evidence of harmful behaviour during child and adult protection investigations, use this form.
If the referral is about a former employee who was removed from a regulated role because they harmed a child or protected adult, use a different form.
