This is an offline application for a PVG scheme disclosure (Existing). You can use it if you cannot complete the application online.

It's for existing PVG members who are continuing to work with the same vulnerable group on their membership. It adds an organisation to their membership.

It can also be used by people who are personal assistants, self-employed or work for an individual.
