Joining up health and social care

The integration of health and social care is the Scottish Government's programme to improve care and support for people who use health and social care services.

Integration aims to ensure better care and support are available for people who live with long-term conditions and disabilities, provided in a seamless and coordinated way.

Users of health and social care services can expect to be:

  • listened to
  • involved in decisions about the care they receive
  • an active participant in how it is delivered
